The Holmdel Police Department frequently receives inquiries from government offices, businesses, and citizens on the regulations and procedures for obtaining an individual's criminal history record. Police Reports

To obtain a police report, please complete the Request for Records form and return to the Records Bureau. The requestor will be notified when the report is ready.

There is no charge for accident reports if picked up in person. To obtain an accident report electronically, for a $5 fee, please visit www.crashdocs.org

Motor Vehicle Crash Self-Reporting Form

Link to State of New Jersey website. Users can download the SR-1 Form. This form is used to file a self-report for motor vehicle crashes that were not investigated by the Police.
